The Patient-Centered Medical Home (PCMH) model is an approach to delivering high-quality, cost-effective primary care. Using a patient-centered, culturally appropriate, and team-based approach, the PCMH model coordinates patient care across the health system. It strengthens relationships between patients and their healthcare providers, improves the quality of care patients receive, increases access to clinical and non-clinical resources, and enhances patient and staff satisfaction while reducing overall healthcare costs.
